A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, heart disease, diabetes, arthritis and stroke.
What is a GP Management Plan?
Priority Care Medical offers GP Management Plans and Team Care Arrangements for our patients. A GP Management Plan is implemented by your doctor to co-ordinate team care arrangements, consisting of Allied Health and Specialists.
Your doctor and nurse will work out a plan that is best suited to your needs. Once your GP Management Plan is in place you will be eligible to access allied health services.
Strict Medicare criteria need to be met and your doctor will explain these to you.

GP Management Plan appointments are provided at Priority Care Medical on Weekdays and can be booked through our Front Reception staff. These plans are conducted as part of a collaborative approach between the doctors and nurses of Priority Care Medical and YOU.
